Urethral stricture occurs mainly in men and may follow injury or repeated attacks of URETHRITIS (inflammation of the uretfral). Now that antibiotics are used to treat urethritis due to infection, urethral stricture is less common than it was in the past. The condition causes painful or difficult urination and ejaculation, and may cause deformity of the penis during erection. Chronic RETENTION OF URINE may develop, leading to repeated urinary tract infections and HYDRONEPHROSIS ( distension of the kidneys with urine). Urethral stricture is an abnormal narrowing of the urethra (the tube that releases urine from the body). In males the urethra also allows semen to pass out of the body during ejaculation. The stricture can cause a partial or complete blockage in urine flow. The narrowing may be due to inflammation, injury, disease or scar tissue from surgery. Frequent urination, urinary tract infections, and inflammation or infections of the prostate and content of the scrotum may occur. It showed a typical severe proximal bulbous urethral structure. A retrograde urethrogram showed that the stricture was about 1cm in length. Left untreated, more severe obstruction of the urethra can result in kidney damage. Urethral strictures are more common in men because their urethras are longer than those in women, and therefore more susceptible to disease. A urethral stricture is a scar in or around the urethra, which is the tube that drains urine from the bladder. This impairs normal urine flow. It is generally caused by scar tissue around the urethra, formed as a result of trauma or infection . A less forceful urinary stream or a double stream usually occurs with mild strictures. The buildup of pressure behind the stricture may cause the formation of passages from the urethra into the surrounding tissues (diverticula). A urologist diagnoses a stricture by looking directly into the urethra through a flexible viewing tube (cystoscope) after administering a lubricant containing a local anesthetic. To widen the urethra, a urologist may dilate or cut (urethrotomy) the stricture. Urethral strictures can recur and may require excision of the scar and surgical reconstruction of the urethra, sometimes with a skin graft. Treatment of Urethral StrictureTreatment entails dilation of the narrowed sectionof the urethra by inserting metal rods of gradually increasing diameter, or removal of the scar tissue with a cutting instrument inserted into the urethra. An extensive stricture may need surgical reconstruction of the urethra.
