Torsion may occur spontaneously or follow strenuous activity, and may result from disorders in developinent of the spermatic cord or the capsule around the testis. Because the testicular artery is in the cord, torsion cuts off blood supply to the testis. Immediate symptoms are severe local pain, nausea and vomiting, followed by swelling and skin discoloration in the scrotum, and fever. Emergency surgery within a few hours offers the only hope of saving the testis. If the cord is untwisted early enough to prevent death of the testis, it is anchored in the scrotum to prevent further twisting. If damaged beyond recovery, the testis is removed. In either case, the testis on the other side is anchored to prevent future torsion. In testicular torsion the spermatic cord that provides the blood supply to a testicle is twisted, cutting off the blood supply, often causing orchalgia. Symptoms are acute scrotal pain and swelling, nausea, and vomiting. Testicular torsion is an extremely painful condition that requires immediate medical attention. Immediate medical attention is needed to prevent permanent damage to the testicle. Testicular torsion is most common between the ages of 10 and 20 but can occur at any age. Testicular torsion can occur after strenuous activity but can also occur during sleep or without apparent cause. If testicular torsion is not treated within a few hours, it can result in infertility or the need to remove the testicle. Testicular torsion is a twisting of the testicles and the spermatic cord (the structure extending from the groin to the testes that contains nerves, ducts, and blood vessels). Torsion can cause shrinkage (atrophy) and tissue death (necrosis), and may require surgical removal of the testicles (orchiectomy) if not treated promptly. It is also believed that torsion occurring during fetal development can lead to the so-called neonatal torsion or vanishing testis , and is one of the causes of an infant being born with monorchism.
